Medicaid and Health Plans
We assist with new Medicaid applications, renewals, and long-term care Medicaid guidance. Services include support with Medicare enrollment for individuals age 65 and older, managed care plan navigation, and understanding coverage options. Additional support may include insurance premium assistance and medication copay assistance when available. We also assist with scheduling medical appointments with primary care providers and specialists.
Food Assistance (SNAP)
We provide assistance with Supplemental Nutrition Assistance Program (SNAP/Food Stamps) applications, renewals, benefit increases, and EBT card activation. Additional food support may include access to frozen food, community kitchen meals, and food donations when available.
SSI and Social Security Programs
We assist with applications and recertifications for Supplemental Security Income (SSI) and Social Security Administration (SSA) benefits. We also provide guidance for related documentation and case updates.
Free Phone Program
We assist with applications for free cellular phone programs and free landline phone services, including options for individuals who are deaf or hard of hearing.
Housing & Utility Assistance
We assist clients with locating affordable housing, applying for housing programs such as Section 8, and accessing support for housing repairs, maintenance, and moving-related needs. Utility assistance may include electricity credits and utility bill forgiveness for electricity and water, available up to two (2) times per year, subject to eligibility and funding availability.
Public Benefits & Financial Support
We assist with benefit recertifications, Federal Poverty Level (FPL)–based credits, and other eligible financial support programs based on individual eligibility.
Basic Needs Support
We provide access to clothing, household items, cleaning supplies, and personal hygiene products to support essential daily living needs when available.
Transportation Support
We coordinate transportation services and assist with applications for disability parking permits and public transportation bus passes.
Internet & Connectivity Assistance
We assist with applications for low-cost internet services and connectivity programs to help individuals stay connected.
